Direct Debit Service Agreement

1. Donations to Amaze Incorporated (Amaze) over $2 are tax deductible unless otherwise identified. Ongoing regular donations are tax deductible and will be receipted after every transaction. 

2. It is your responsibility to ensure that sufficient funds are in the nominated account when the payments are to be drawn. If sufficient funds are not available on the next debit day, and you have not notified Amaze, your financial institution may deduct a fee from your account. This fee varies with each financial institution and will not be reimbursed by Amaze. 

3. By giving your payment details to sign up to a regular gift you are authorising and requesting Amaze to arrange for funds to be debited from your account at the financial institution identified on the 1st day of each month. All details of the accounts to be debited have been supplied. 

4. Your records and account details will be kept private and confidential to be disclosed only if requested by you or your Financial Institution if a claim is made for an alleged incorrect or wrongful debit. 5. You may change the arrangements under a Direct Debit Request by contacting us on 03 9657 1600 or [email protected]
    5.1 If you wish to stop or defer one of your contributions you must notify us at least five (5) business days before the next debit day.
    5.2 You may also cancel your authority for us to debit your account and cancel your membership at any time by giving us fourteen      (14) days notice before the next debit day. 

6. Complaints, grievances and disputes will be treated seriously, and in a timely manner, having due regard to procedural fairness and confidentiality. If you believe that there has been an error with your donation, you should notify us directly in writing or by email at [email protected] as soon as possible so that we can resolve your query.
